Memory care — a secured RCFE endorsement — runs $5,200–$8,500. Skilled nursing facilities average $9,500–$13,500 for a private room. In-home care through a licensed Home Care Organization runs roughly $28–$42 an hour.

Geography matters within the county. La Jolla, Del Mar, Rancho Santa Fe, and Coronado run 12–20% above the county median on comparable care. Chula Vista, El Cajon, National City, and South Bay cities tend to run 8–14% below. North County communities — Carlsbad, Oceanside, San Marcos — sit near the county midpoint.

What's included — and what isn't

A base RCFE rate usually covers housing, three meals, 24-hour staff, housekeeping, laundry, and activities. What gets billed separately — medication management above a basic tier, two-person transfers, incontinence supplies, and one-on-one aide time — is where quoted prices and real bills diverge. Always get a full itemized rate sheet before comparing communities.

California's RCFE licensing rules (Title 22, §87572) require facilities to disclose their rate schedule in writing. If a community is vague about add-ons, that tells you something important.

Programs that lower the bill

The biggest levers are a shared room, choosing a smaller board-and-care home (six beds or fewer) over a large campus, and right-sizing care level to current need. For those who qualify, the Medi-Cal Assisted Living Waiver (ALW) covers the care portion of RCFE costs — San Diego County has an ALW slot allocation and a wait list. VA Aid & Attendance can add $1,800–$2,900 a month for eligible veterans at the VA San Diego Healthcare System (La Jolla campus).
