California's IHSS program pays for in-home care for Medi-Cal-eligible seniors who need help to safely stay at home. Here's what San Diego families need to know.

In-Home Supportive Services (IHSS) is a California Medi-Cal program that pays for domestic services (cleaning, laundry, meal preparation), personal care (bathing, dressing, grooming), and paramedical services for eligible seniors who live at home. IHSS can also pay for protective supervision for those with dementia or behavioral needs — a benefit that's often overlooked.
In San Diego County, IHSS is administered through the San Diego County Health & Human Services Agency. An IHSS social worker visits the home to assess approved hours; monthly hours can range from a few dozen to hundreds depending on the level of need.
Eligibility requires full-scope Medi-Cal (Medi-Cal, not just Covered California), California residency, and a medical need for in-home assistance to avoid institutionalization. Age is not a strict requirement — IHSS serves disabled adults of any age — but seniors are the largest group served.
Apply online through BenefitsCal.com or in person at a San Diego County HHSA office. Once financial eligibility is established, an IHSS social worker schedules a home assessment. Approval and the first paycheck for a provider typically take 30–90 days.
IHSS can work alongside other programs. A senior receiving IHSS for in-home help may also be on the Medi-Cal ALW wait list for eventual RCFE placement. IHSS cannot be used inside a licensed care facility (RCFE, ARF, or nursing home), but it can bridge the gap while a family waits for a Medi-Cal waiver slot.
