California's ALW can cover the care portion of RCFE costs for qualifying Medi-Cal seniors. Here's how San Diego families apply, what to expect, and common mistakes to avoid.

The Medi-Cal Assisted Living Waiver (ALW) is a Home and Community-Based Services (HCBS) waiver that pays for personal care, medication management, and many support services within an RCFE — but not room and board (rent and meals). Eligible seniors continue to pay room and board out of pocket, from Social Security income, or through a Supplemental Security Income (SSI) supplement.
The ALW is not available in every county at all times; San Diego County participates in the program, but enrollment depends on available waiver slots. There is frequently a wait list, so applying early is essential.
Eligibility requires full-scope Medi-Cal (not just emergency Medi-Cal), a nursing-facility level of care established through a DHCS assessment, and residency in a participating RCFE that accepts ALW residents. Income and asset limits apply under Medi-Cal rules. Working with a Medi-Cal eligibility worker at the San Diego County Health & Human Services Agency (HHSA) is the standard starting point.
San Diego County Aging & Independence Services (AIS, reachable at 800-339-4661) can screen families for ALW eligibility and refer them to the appropriate HCBS waiver coordinator.
Families often wait until a crisis to apply, missing the window to get on the wait list. Others stumble on the asset rules or don't realize their RCFE doesn't accept ALW residents. An elder-law attorney or a knowledgeable senior advisor can help structure the application and time it correctly.
